We understand that your time is precious and that you need an optometrist who is professional, yet convenient for your lifestyle. In order to make the transition to our office smoother for you, we have created a New Patient Center with everything you need to get started.
In an effort to make you more comfortable when you arrive at our office and to streamline the new patient process, you will be able to complete all the necessary items prior to your first visit. At InVision, we strive to make sure the time we spend with you during your visit is quality time.

SCHEDULING AND WHAT TO EXPECT
We at InVision try to accommodate your busy schedule with the best day and time for the exam. Please note, since Dr. Cohen is highly sought out, our first available appointment could be up to two months out. We do have a cancellation list to be added to in the event that a sooner appointment becomes available.
INSURANCES WE TAKE
Vision: VSP, Spectera, Superior, Eyemed (not all plans) and Davis
Medical: Blue Cross Blue Shield, United Health Care, UMR, GEHA, Cigna, Aetna, Medicare, Tricare
Medicaid: Blue Cross Blue Shield Centennial, and Western Skies
Our office is going paperless. In order to help us achieve that goal, we have asked that all patient forms be filled out online. If you are a new patient, click on new patient forms and then new patient page. If you are an existing patient, you are more that welcome to call the office to get your username and password. Fill out each that that is relevant to the patient. If you have questions or need help with the forms, don't hesitate to call the office.
